INCIDENTAL ACUTE APPENDICITIS DUE TO ENTEROBIUS VERMICULARIS: CASE REPORT

Abstract

Introduction: Acute appendicitis, often caused by the parasite E. Vermicularis, poses a unique challenge for surgeons. Efficient diagnostic techniques are crucial for optimal management of this surgical emergency Objective: Identify E. vermicularis in surgically removed appendices, correlating its presence with symptoms and histological findings. This research sheds light on its potential role in the development of appendicitis Case reports and methods: We conducted a literature review, including recent guidelines, trials, case reports, and original articles from databases: PubMed, UpToDate, Scopus, Embase, and the Cochrane Database of Systematic Reviews. With analyzing a case from Military hospital's surgery department, addressing the challenges of enterobius appendicitis Case Report: 20-year-old male presented with acute appendicitis, featuring right lower quadrant pain and anorexia. Blood tests showed a WBC of 15,500/mm3 and elevated CRP levels. Ultrasonography confirmed an inflamed appendix, leading to an urgent laparoscopic appendectomy. Macroscopic examination revealed intraluminal pinworms. After a single Albendazole dose, the patient fully recovered in two days, with another dose scheduled for two weeks later Discussion: Identifying acute appendicitis caused by E. vermicularis is challenging due to its similarity to typical appendicitis symptoms. Once confirmed, choosing the right management method, often involving appendectomy with specialized techniques, is vital to cure the condition, minimize complications, and prevent peritoneal contamination. Anthelminthic treatment is administered as well Conclusion: Choosing the right surgical technique for acute appendicitis requires identifying the underlying cause, such as E.Vermicularis. The decision should be based on a thorough evaluation of the advantages and disadvantages of each approach, considering the patient's health and severity of the underlying problem
